[reccom] Change in review (voting) form

In order to prevent the votes and comments from being submitted if you
hit the "enter" key in one of the non-comment fields. I've changed the
form so the date field is at the bottom and you have to fill it out
before you can submit your votes.  I hope this will be helpful.  

Please let me know if you have any suggestions on how we can improve
how we do things.
